The Mucocele of the Apendix with Rseudomyxoma Peritonei

Abstract

|
Pseudomyxoma peritonei is a process in which mucinous material is found free within the peritoneal cavity. This is commonly associated mucoceles of the appendix, ovarian cysts, or mucous secreting carcinoma of the gastrointestinal tract. While in many cases the primary lesion is clearly either benign or malignant, considerable confusion exists regarding mucoceles of the appendix with pseudomyxoma peritonei.
Mucocele of the appendix would appear to develop as a result of a low grade obstruction in either the appendix or large intestine. A very delicate balance is necessary between the rate of development and degree of obstruction which results in the mucocele formation. As long as the mucocele remains intact, the condition is benign.
When the mucocele ruptures, mucoid material spills into the peritoneum. It is not known if the implants which result are due to cells from the mucocele which grow on the peritoneal surface or if the columnar cells represent metaplasia of the peritoneal cells as they react to the mucoid material.
It is of interest that the implants are able to grow, invade locally, and frequently are much larger than the appendiceal lesion.
Four instances of pseudomyxoma peritonei, secondarly to a ruptured mucocele of the appendix, have been presented with a brief review of literature.
